This position is accountable for independently managing the respiratory needs and care of complex medical patients in their home setting. These patients include home ventilator, trached, oxygen dependent, apnea monitored and suction dependent patients being discharged from hospital based NICU, PICU, Rehabilitation, General Medicine floors as well as Hospital based Clinics i.e. Pulmonology and Charlie Mitchell. The incumbent is expected to teach and deliver patient care at a proficient level in an efficient manner. The position also serves as a resource to others within the scope of the Respiratory Care Practice Act and CHCC’s Home Care policies and procedures. This involves transferring the skills needed to a parent or guardian that will enable that person to be the caregiver for a very complex medical child in an independent setting. The incumbent should be able to schedule their day, work independently, coordinate schedule within an organized interdisciplinary team, as well as perform independent equipment management and selection. The Home Care Respiratory Care Practitioner position requires coordination of services and equipment authorization with physicians, insurance carriers, case management, distribution and CHC Billing team. The position requires extensive travel to meet client needs.
