Facilitate and coordinate admission to the inpatient rehab program. Deliver support and advocacy for patients and families in the program based on unique and individual needs. Facilitate improvement of internal systems, process, and services that support patients in the rehab program. Assist and coordinate CARF accreditation surveys, performance improvement projects, and ongoing program evaluation for compliance standards and optimal outcomes. Efficient communication and care coordination in collaboration with a multidisciplinary team providing high quality services along the continuum of care. Educates patients, families, caregivers, and all stakeholders about program benefits and services. Fosters and maintains relationships with referral sources, outreach communities, and stakeholders while maintaining a high standard of customer service. Maintains data integrity while participating in developing clinical decision-making with leadership.

The Pediatric Inpatient Rehabilitation unit specializes in long-term rehabilitation care and is one of only two California pediatric CARF-accredited facilities. We cater to a wide range of conditions, including traumatic brain injuries, multiple traumas, trach/vent patients, spinal cord injuries, stroke, orthopedic conditions, Guillan-Barre, and other neurologic disorders. Additionally, we offer specialized care for medical patients admitted to the hospital, leveraging our team’s exceptional expertise in an interdisciplinary setting. Our large playroom features two TVs for family entertainment and wall of cabinets filled with toys for patients. We also have a spacious kitchen area equipped with 2 ovens and an industrial-sized fridge and /freezer for families to use during their extended stay, ensuring their child is fully prepared for a smooth transition back home. Our commitment to patient success is evident in our focus on providing specialized education to empower patients to care for themselves before they return home. To ensure we maintain high standards, we utilize a national database to track patient outcomes, with Valley Children’s Rehab patients showing some of the best results in the country, particularly in achieving greater independence after our care.

EXPERIENCE:

  • Minimum three (3) years related experience required.
  • Minimum two (2) years clinical experience in rehab preferred.
  • Experience in admissions, discharge planning or case management.
  • Experience in admissions, discharge planning or case management preferred
  • Knowledge of insurance industry, including different carrier types and case management.
  • Proactively identifies and addresses challenges with minimal oversight and demonstrates skills in critical thinking, problem solving, and projects requiring attention to detail and specific timelines
  • Bilingual skills preferred.

EDUCATION/LICENSES/CERTIFICATIONS:

  • Completion of an accredited nursing program, Bachelor's in nursing preferred
  • Registered Nurse License (RN) required.
  • Basic Life Support (BLS) within 30 days of hire or transfer into position required.
  • Certified Rehabilitation Registered Nurse (CRRN) or Certified Pediatric Nurse (CPN) preferred
